The 41-year-old was charged by NSW Police on Friday with offences including dangerous driving occasioning death, dangerous driving occasioning grievous bodily harm and three counts of cause bodily harm by misconduct.
Police claim the man was behind the wheel of a four-wheel-drive when it collided head-on with a station wagon on Razorback Road, east of Sofala, on February 13.
A 64-year-old woman in the four-wheel-drive died at the scene.
Four others were airlifted to hospital in a serious condition, while a further five passengers were taken by road to hospital. The man, who was issued a licence suspension, is set to face Mudgee Local Court on June 22.
